The information we collect

  • Contact details — your name, phone number, email address and location or suburb.
  • Participant information — details about the person requiring support, including their NDIS funding and how their plan is managed.
  • Sensitive and health information — where it is relevant to delivering therapy or behaviour support services, and only with your consent.
  • Enquiry information — the details you provide when you contact us, complete a form, or submit a referral.
  • Website and advertising data — information collected automatically through cookies and similar technologies (see “Cookies and tracking” below).

How we collect your information

We collect information directly from you when you:

  • complete a form on our website or a referral form;
  • submit an enquiry through a Facebook or Instagram lead form, or another social media platform;
  • call, email or otherwise contact us;
  • are referred to us by a Support Coordinator, health professional, family member or carer (with appropriate consent).

When you submit an enquiry through a Facebook or Instagram advertisement, the information you provide is collected through Meta Platforms’ lead form and passed to us. We then handle it in accordance with this Privacy Policy.

Why we collect and use your information

  • to respond to your enquiry and contact you about our services;
  • to assess eligibility and provide Occupational Therapy, Positive Behaviour Support and related NDIS supports;
  • to manage referrals, appointments and service delivery;
  • to meet our obligations as an NDIS provider and comply with applicable laws;
  • to improve our services and, where permitted, to send you relevant updates (you can opt out at any time).

Consent and sensitive information

We only collect sensitive information (including health information) where it is reasonably necessary for our services and where you have consented, or where the collection is otherwise permitted or required by law.

Disclosure of your information

We do not sell your personal information. We may disclose it to:

  • our clinicians and staff involved in your care;
  • third parties involved in your supports, such as your Support Coordinator, plan manager or other health professionals — with your consent;
  • the National Disability Insurance Agency (NDIA) or the NDIS Quality and Safeguards Commission, where required;
  • service providers who help us operate (for example secure client management, email, hosting and communications providers);
  • others where required or authorised by law.

Cookies and tracking

Our website uses cookies and similar technologies, including the Meta (Facebook) Pixel and analytics tools, to understand how our website is used and to measure and improve our advertising. These tools may collect information such as your device and browser, the pages you visit and the actions you take. You can control cookies through your browser settings. Information handled by Meta is also subject to Meta’s own data policy.

Storage and security

We take reasonable steps to protect your personal information from misuse, loss, and unauthorised access, modification or disclosure, including through secure systems and access controls.

Accessing and correcting your information

You may request access to the personal information we hold about you, and ask us to correct it if it is inaccurate, out of date or incomplete. Please contact us using the details above.

Overseas disclosure

Some of our service providers (for example technology and advertising platforms) may store or process information outside Australia. Where this occurs, we take reasonable steps to ensure your information is handled consistently with the Australian Privacy Principles.
